yeah...just having flash backs to a former job. Good ones actually.
I worked for a software house for 4 years as a programmer of small routines/tester/debugger. I did not have a degree in Comp Sci. but I was a good amateur programmer with business experience and specialized also in report writing using Crystal reports (something the programmers there hated to do, they considered that "prettiness issues"). I was 47-51 at the time my boss was 25-29 and my partner (we were a team of 2 in the debugging end) was 19-23. I was third oldest in a company of 25 and it took me a year to get some credibility I loved that job but they sold the software company to another larger house and they chopped our group entirely after 6 months...I existed as a temp contractor for report writing but I knew the writing was on the wall as I could not compete with those with degrees and eventually the contracts disappeared and I moved on to another career
My best guess is there is a bug, that is extremely rare and triggered by certain actions that almost no one takes. So in other words, alter your method since it is likely the method that is causing the problem.
When I upload attachments, I click the paperclip icon on the toolbar, attach the files, embed them, close the attachments window, and submit the post. I've never had anything disappear in 5 years.
First off this appears to be a time based issue. If you upload the images and then spend more then 10-15 mins typing the images appear to dump when you save. If you go back in and edit the post, you see they are gone. Now in the edit if you quickly add the photos back and reinsert them they show.
So the work around for me is to type up text and then add images just before I save.
I am not sure how I can confirm it any more than I already have but I agree with @tturner86 it appears to be time based.
I already suggested that I think there must be a temporary place where these images are stored prior to the message is finally submitted. I think that is where the problem lies.
Not having access to the code, this makes sense to me. This is what I think is happening:
the attachment files would be held in the temporary file and IF the post is submitted....then the attachments are archived...if the message is abandoned then the images are discarded.
When someone (especially me who posts several messages in a large post) immediately uploads the images and then starts to compose the post, embedding as you reach a certain point in the post...as long as this temporary file is constantly used all is well.
BUT if you embed an image first, then take 10 or more minutes to compose the rest of the message, there might be a timeout function where the temporary file area believes the message has been abandoned and it empties its contents.
Perhaps as @Big Mike has said the images might be still cached on the user's computer and so they think the imaged are still there...perhaps not.
But that is where I think the problem is....a temporary file that is not archived when the post is submitted since it aborted its contents thinking the message was abandoned.

Here is what I will do to test that there is a temporary file in effect. I will start a message here, add an image .... do a preview of the message....take a screenshot of the preview and message to show the image and the embedded code. So everything is there except the message has not been submitted.
then I will go to my attachments and see if there is a record of that attachment. my screen shot will include the time of day. If I am right about there being a temporary file then that attachment won't appear in my attachments.

I guess you would have to do it yourself to confirm I am not messing with you.
Both of you, just start a post. Attach files. Wait 30 minutes and see if when you submit, the images are there. If you think 30 minutes is not long enough, wait 60 minutes. Make sure you wait at least 30 or 60 minutes (not 29 minutes).
